Demographic information

Let's start with a few basic questions about you.

Please ensure the full name you enter exactly matches the one in your government ID.
3. Your gender
Data is collected and used to build inclusive AI models that do not have an unfair bias.
4. Your date of birth
All participants must be 18 years of age or older to complete this form and participate in this study. Data is collected and used to build inclusive AI models that do not have an unfair bias.
We may need to contact you by phone for your session.

Building an unbiased and diverse future

The next few questions are about your phyisical appearance. Some might seem quite personal, like skin tone or hairstyle.

We collect this information so the avatar technology our client is developing can capture the diversity of all of us. This is an excellent opportunity to make sure the technology of the future works for everyone, including people who look like you!

7. Your ethnicity

10. Uncommon characteristics

Do you have any specific minoritarian (uncommon) characteristics, you feel are not currently available or up to par with regards to avatar technologies that you feel you could help with. Example - missing limbs, dwarfism/little person, Siamese twins, paraplegic, vitiligo, braces, facial piercings, visible tattoos, etc.

Uncommon characteristics

11. Important questions about your eyesight

For this project, you will need to be able to read text around 6-7 feet away displayed in a size people with good eyesight can see.

Glasses cannot be worn during the session, contacts are fine. If you need to be wearing contacts to read text this size, please ensure you have put them on prior to attending your booking.

If your eyes cannot read text of this size, but are still interested to participate, do still complete the form. Should this requirement ever change, we will gladly contact you to let you know.

Eye capabilities
12. Eye sensitivity
Different types of lighting and brightness will be used during the sessions. At times, the lighting may be extremely bright to ensure proper video captures.

Understanding the Avatara Project

During the session, you’ll mimic body movements while wearing various outfits and accessories, read scripts aloud, and more. Your participation helps train AI avatars to behave more naturally.

One part of the study requires you to perform basic movements wearing only undergarments.

13. Wearing undergarments

Do you own undergarments similar to those shown here, and would you be comfortable wearing them as one of the outfits for the study?

By similar, we are referring to the coverage of the garments, not the colors.

Wearing undergarments

14. Different outfits

Do you have a variety of at least four available outfits, including ones that would be considered seasonal, athletic, for special events, or other very different looking outfits that you could bring to use for the study?

This includes the outfit you wear to the location, as well as the undergarments outfit described above.

Different outfits
15. Do you have a history of epilepsy, seizures, or severe traumatic brain injury (i.e., concussion with nausea and/or memory loss)?
The technology used to capture movements might expose you to blinking lights.
16. Do you have any health or mobility issues that could prevent you from spending time on your feet, walking, squatting or doing some age/body appropriate exercises.
17. Are you claustrophobic?
Please answer yes if you can have issues being in a small closed room. A portion of the session would require you to be in a capture dome that is closed.

Availability questions

18. If you live close to the Fremont area and have free time, are you willing to drop by in case we have a free spot?

We may have unexpected open spots on the collection days. We are looking for people (Back-ups) willing to come to the venue to see if any spots would be open.

  • You would need to be free for 4 hours.
  • If there is an open slot, you would become a Primary Participant, and complete the entire session.
  • If none are available, you will be sent home within about 2 hours and paid $50 per hour for your time you've spent on site.

If you cannot stay for the duration you are needed on site, no payment, even partial, will be made. Once you become a Primary Participant, you will no longer be able to come in as a Back-up.

Whether you are a Primary Participant or a Back-up participant, you are expected to arrive on time, a little early is recommended. If you arrive after the session start time, we may not be able to consider you, and if so, no payment will be made.

Primary / Backup preference

19. Advance notification

How much advanced notice would you need to come to our Sponsor's venue in Fremont (zip code 94555)?

We will do our best to book everyone we can, with as much notice as possible. Occasionally, we may get cancelations and need to fill those spots very quickly.
